Not usure if you have this covered or not, but worth mentioning:
aside from the logical normal stuff like contacts, date, numbering,
description and fees, delivery, etc., be sure to include terms that define
just what the client is purchasing, rights and ownership, and what rights
you keep as creator. It's especially important if work was done without a
written agreement.

Something like:

Terms:
Due Date
Delivery

Ownership
(Example: All original artwork remains the property of the artist, except as
noted:...)

Rights
(Rights transferred are limited to:...All other rights reserved. Rights
specified herein and ownership of mechanicals transfer to client upon full
payment of all fees and costs.)

Credits
(if applicable, how your credit line should read when published. Credits may
be required for photos/artwork/quotes as well.)
