Sorry. The HCFA-1500 (or RRB-1500) is the standard paper health
insurance claim form. This form is what the physician's office fills out
and sends to your insurance company to get paid for the procedures
they did to treat your diagnosis.
They are used everywhere, which is why I thought to ask if someone has
already built an xml schema for the form.
