If you mean a favicon (the icon that appears on the favorites menu when
the page is added) then create a standard 16x16 icon and name it as
'favicon.ico'. Put this in the root directory of your site.

For it to work in all browsers (particularly IE) it needs to be saved in
the (windows?) icon format created by programs such as Any to Icon -
http://www.aha-soft.com/anytoicon/. Firefox will render just about
anything as a favicon, however some browsers are a little more picky.
