Author "post-it" with text and lift-up corner

I have to place a vector element looking like a post-it (with annotations)
pasted on another document.
I want to apply some distortions (applied to the yellow rectangle AND the
text) to indicate that one corner is lift-up and a realistic drop-shadow
(i.e. a shadow having a variable width depending to the dustance between the
post-it and the background image).
I guess this is not different than giving the effect of a lifted page in a
book.
I'm sure this exists somewhere. Any idea ?
Thanks

Not really what I'm looking for:
I want to apply the curved effect to both the "paper" AND the text.
I can't do that with the template you indicate.
Thanks however.
