Hard to tell, but it looks like that image was created by an artist, not
a computer program. If it started out as a photograph, it was traced by
a skilled artist.
If you're looking for "click this button, run this filter, click this
other button, and that's what you'll end up with," I'm afraid you're
likely to be disappointed.

It looks like a combination of effects to me - a photo treated with
posterize (image > adjustments > posterize - try a small number like 4 for a
more stylized effect, higher numbers for more detail) combined with artwork
probably done in Illustrator or Freehand.
The background uses a halftone effect - I've created a very similar look
before, but I can't remember exactly how I did it - maybe someone else can
fill in the blanks.
