Need an email addressbook to display on a website

I need to replace a CGI email address "book" with something else
because my host is going to drop CGI support soon.
I have already been to http://www.cobra-scripts.com and picked up a
free message board and a free email form and they are great
replacements for the CGI scripts that I had on the site. The cobra
scripts came as a single .php file that I ftp'ed to the site, open in a
browser, did just a little editing, and they work great. I need
something like that (free and simple) to replace the email address book
too because I'm not a wizard with PHP.
It doesn't have to be PHP either---it just has to be free, easy for me
to set up and maintain and allow the audience to input their own data.
Just so everyone knows that this is not a ruse by a spammer to collect
email addresess, please go to www.campbarbee.com and click on the
"email addresses" link at the top of the page. You can see for yourself
the CGI that I need to replace. The campers and counselors use this
"database" to leave their email addresses for their friends to find and
use. (If you click on the link "message board" you can see the free php
that I got from cobra-scripts.com too.)
All I need is First Name, Last Name, and Email Address. Layout is
irrelevant, rows or columns would work fine. Nothing fancy, just a
place where people can input their data if they want to and see the
data that everyone else has entered. That's all I need.
Any ideas would be helpful! Thanks!

Wait. So, rather than find a new host, you're willing to replace a core
piece of functionality on your site? That's crazy talk

My thoughts exactly. Just change hosts, amend the DNS records for the
domain, and voila.

no problem
first, backup all your stuff.
then, signup with someone who rates ok at http://www.webhostingtalk.com/forumdisplay.php?f=4
rates ok as in, find something then search the name of the host there
too ;)
signup, upload.
go to your registrar and enter the new nameservers.
load a bong
watch a movie
by the ending, you won't have any more problems.
