Author Form for uploading a file to a server

Hi!
I am a DUMMY when it comes to programing, I need help!

This is what I want:

A form where the user can send both text info and upload a image!
I dont want a script that uses the users own email client to send it.

I would be very gratefull if someone could give me a clue as to what I need to do! :)

If this question belong in a diffrent forum, let me know! :)
