Is there a way to do a Google image search where my keyword is in the
filename only? Ignore keyword in url or alt tag.

The allinurl: operator works under images as well. Although I don't know
how to restrict it to *just* the filename.

BTW, Opera users (and perhaps GreaseMonkey users) can check out a script I
wrote which "improves" Google Images, by removing a lot of the "fluff"
displayed with results, and bypassing Google's frameset image viewer (eg.
clicking on the image goes *directly* to the image).

This works:

allinurl:*sock puppet.jpg

You would have to try different file extensions, but I imagine .jpg would
take care of most.

That's a bit hard :-) URL contains the filename.

Use the complete filename, or inurl:keyword

Well the web know nothing of files and filenames - it's just a URL.
