How do I set up Godaddy ASP Form-Mailer in Publisher?

Using Publisher 03 I would like to use ASP Form-Mailer provided by Host
Godaddy. What do I enter in form properties to make this work? Link to their

Somehow I doubt that you will be able to do this, but perhaps you will read
