is anybody know How to set up a form from Macromedia Dreamweaver MX 2004,
that user can type in and the date will go to Access database, please help

You should ask this in a Dreamweaver newgroup...this one's for MS Publisher,
so you're unlikely to find it on the MS server. Try; macromedia.dreamweaver
on your newserver.
