I have created a comment card with Publisher and now would like to e-mail it
to clients and have then respond back to me with a few yes or no answers. Is
this possible?

The easiest would be to output the comment card from Pub to a .pdf then
create a fillable pdf form save it and email it as an attachment. You'll
need Acrobat (not the reader) or perhaps one of the free pdf creators
(primopdf, cutepdf...not sure if you can create forms with the free ones,
but you can check it out).
