I have created form and it has drop down menu box. This box is used to
select country and I have populated with few names however user should
be able enter name of the country in this box if country is not
listed. How do I add this option in the same drop down menu? Any help
will be highly appreciated

This is not possible with dropdown form field. You will need to write some script that when the user
select "other" that they then fill in a text box.
