submit button driving me crazy

frontpage 2002

this page works sometimes and not others
i have tried many things but still cannot get the submit button to function
all i want it to do is allow the customer to fill in the form and email the
results to my cousin yocumblinds@comcast.net
currently i have it set to email me but nothing happens
i tried confirmation page, form changes, email changes, etc.
i would appreciate any help

| The email address must be under yocumblinds.com, as in dennis@yocumblinds.com
