| What did your host say when you asked them to check the server's application
event log?
Do you mean ASP server extensions or FrontPage server extensions?
Is this a basic FrontPage generated form that is supposed to have the
results sent by email? If so, did you rename the file extension to .asp
from .htm? If so, it won't work.
Did your host configure the FP server extensions forms to email transport?
