Author Response Form (Newbie Question)

I've recently set up a company Web site that contains a template FP response form. When I try to submit the form I get a FrontPage Run-Time Component error that says the Web server needs FP Server Extensions to function properly.
My service provider has FP Server Extensions installed and the other components of the site that need Server Extensions work properly.
Anyone with any ideas? Thanks in advance.
