Dreamweaver - Need help on form.

Need help on form.

I made a submition from but i need help w/ the CGI to make it work. if anyone
knows how to do this or some helpful pointers. i attached a copy of it before,
please someone help me out.

Go to http://www.cgi-resources.com and find a CGI script called Form Mail ..
very clear instructions on how to use it.
