HELP! Looking for a script

So I have this online form that people come to and fill out hit "submit" and
the form is sent to an individual as an e-mail. if anyone wants to see that
form it is here

and it will already have the text from the previous nice looking page, that
way the person hits submit on the pretty page and it is sent as an e-mail. and
then that person goes to the print friendly page already filled out and can
print it so there is a paper version.

I'm trying to do what I just explained. What kind of script do I need?

Ask your host what the provide for processing forms.
