Probably. But there are other reasons; back in the day before I had
reasonable hosting, I forwarded my domain to my local ISP, and in order for
all the pages to use my domain name I had to "lock" it. (It was horrible -
it even locked outgoing links off-site.)

But presumably frames are much more common.

Is it normal practice to request permission to link to URLs which are
accessible on the web?

My experience is that websites usually welcome any and all links to them
from elsewhere.

Some sites get cranky about some links - see, for example:

http://www.wired.com/news/politics/0,1283,52213,00.html

More can be had by doing a web search for [deep linking]

Just as an aside, I've been working, on all the sites I administer, to
make all external links
a) open in a separate window
b) have one of the little external linky graphics
so that it's clear that it's not my content.

The box with an arrow pointing out of it, indicating that the link goes
offsite. Wikipedia, among others is making extensive use of it, although
they're using CSS3 selectors, so folk using IE won't see them. I just
set a class. The image is:

That's overkill and not really a necessary reason to create pop-ups IMHO.
