I need to insert the URL where the visitor come from in to the form.

For example I have page 1,2,3 and 4 linking to the form page. Now I like to
insert 1,2,3 or 4 depends where the visitor come from in to the form.

How can I do this? I mean there should be a pssibility with server variable
or so but I don't have an idea how to do this.
