I would like to create a form whereas when a user is finished, they can choose
multiple email addresses to send it to. We want a time off request form that a
user can fill in name, dates, etc. and THEN pick from or be able to enter THEIR
supervisor's email address. THEN Submit the form and have the form information
be sent to the email address or supervisor they choose. With MS Server
extensions, I know how to send any mail from a site to ONE email address but
not the ability to choose what address to have it go to during the form input.
Has anyone ever heard of doing this? Any ideas?

What's your server language? easy to do in asp vbscript using cdo or cdosys
mail script.

I am a networking person, not a coder so 'simple' is not to me. Do you know of
any extensions that can be purchased or downloaded that will do this? I did
not find anything at the Macromedia Dreamweaver Extensions site.

Ok what you are looking for is a custom script. There are many free scripts
that will do what the FP Server Extensions do but they won't allow the option
from what I know. What I would suggest is that you either get someone to do
the whole job which isn't much. Or download a script you like and post the
script here and there should be someone who would be willing to tell you how to
modify the script to meet your needs.
