Author web forms - need help

I have created my form in DWmx2004 and uploaded it to a website on a linux
server..the next bit i am having trouble with..Can anyone simplify the task of
finding and implimenting a script for me. the form can be seen on
http://www.firecraka.co.uk/easy_quote.htm i just want to complete the next
steps of making it work.
Would be grateful for any help on this.

What server-side scripting languages does the server support? ASP,
ColdFusion, PHP? PERL CGI scripts?

I am hosting with fasthosts on a linux server for this particular page. The server uses PERL cgi scripts.

Then I'd recommend the NMS Formmail script, available at
http://nms-cgi.sourceforge.net. I believe there are instructions and
tutorials on the site, and posting here with specific questions should get
you some help as well. Installing it isn't a complete no-brainer, but isn't
as hard as it might initially seem either.
