searching for store locations

| I have a question about pages where the user can enter a zipcode and find
locations of the store. I want to be able to do the same on another site.
How would I do this, or is there a ready-made application that would have
this function?
Example: a user enters 92084, and the headline on next page reads something
like Local Plumbers in VISTA, CA, then lists the nearest locations, etc.
