Dreamweaver - searching for store locations

I have a question about pages where the user can enter a zipcode and find
locations of the store. I want to be able to do the same on another site.
How would I do this, or is there a ready-made application that would have
this function?

Example: a user enters 92084, and the headline on next page reads something
like Local Plumbers in VISTA, CA, then lists the nearest locations, etc.

We use the zip code database from the following site.
