Does this happen after installing an extension(s)?
If you want to stop it from happening, you can modify the config file that specifies the default visibility of that toolbar.
First look here:
C:/Documents and Settings/username/Application Data/Macromedia/Dreamweaver MX 2004/Configuration/Toolbars/toolbars.xml
and if it isn't present, then look here:
C:/Program Files/Macromedia/Dreamweaver MX 2004/Configuration/Toolbars/toolbars.xml
Open up the file, and look for a toolbar tag with an id of Standard_Toolbar. Change the initiallyvisible attribute from false to true. Save the file, restart DW, and you should be fine from there.
