Author DW4 crashes, checkin/checkout, index.html

I have a new client. I downloaded the entire site to my (Windows 2000 Pro,
Dreamweaver 4) PC using WS_FTP Pro. I then launched DW4 and defined the site.
All my usual preferences and standard setup, except that I enabled
checkin/checkout.

I then clicked on the index.html file to edit it. DW4 crashed.
Relaunched DW4 and same results.
Opened file in TopStyle and had no problems with it. Saved it. Attempted to
open with DW4 and crash.
Created new file and named it temp.html.
Opened index.html with TopStyle and copied the entire contents into the
temp.html file.
Successfully opened temp.html with DW4.
Renamed index.html to saved20040430_index.html and renamed temp to index.html.
Successfully open the (new) index.html with DW4.
Uploaded index.html to the webserver.
Clcked on index.html to open it with DW4 and Crash!

Repeated the entire process with checkin/.checkout turned off and no problems.

Repeated the entire process with checkin/checkout turned on only used
index2.html as my file name instead of index.html and no problems.

Does anyone have a clue as to what is happening here?

I only have the crash when checkin/checkout is turned on and only with the
file named index.html

is Contribute 2 or MX 2004 in use by anyone on this site?

The answer is: Contribute 2 screws up files in the _notes directories and
causes DW4 to crash.

Long discussion in the Contribute Forum under the title of: Conflicts with
Dreamweaver 4.0 & Contribute 2 Trial?

Thanks for pointing me in the right direction with the question about CT2.
