Author forms that tabulate data

Is it possible to create a form in Dreamweaver that tabulates data? I need to
have a questionnaire where the responses are on a scale from 1 to 5, depending
on the results it will bring you to an explanation of your results (just like
the typical Cosmo quiz). If it is not possible, can you point me in the
direction of where I could go to do that without spending a fortune on a
database? Thanks, Johanna

Check out the form calculator from www.yaromat.com
Keep in mind that any users without JavaScript won't have any calculations.
You don't need a database for calculations, though you do need some kind of
server language. Having the server do the math means the calculations will
work for everybody.

Okay, I downloaded the extension. Is there a tutorial or anything that goes with it?

No, but the dialog box works just like a calculator. It shouldn't be too
much of a pain to use.
